Abstract
This chapter examines investigation Decision Logs or Policy Logs as formal records that document key decisions, rationales, and contextual information, thereby ensuring accountability, transparency, and compliance with the Criminal Procedure and Investigation Act 1996 and the National Decision Model. It explains that Senior Investigating Officers retain responsibility for recording, reviewing, and validating entries, while emphasising precise, contemporaneous documentation to support later scrutiny, disclosure, or case review. The chapter outlines the structured application of the 5WH framework, detailing who records decisions, what constitutes a significant entry, and how chronological numbering, timing, rationale, and available information collectively establish an auditable decision trail. It further distinguishes between major decisions recorded in Policy Logs and routine actions maintained in casebooks, while identifying essential content areas such as enquiry management, administrative structures, inter-agency coordination, and risk oversight. Finally, the chapter underscores that accurate, contextualised record-keeping protects both investigators and organisations by evidencing reasoned, proportionate decision-making under operational constraints.
