Abstract
A 9-year-old girl presented with multiple punctate plantar keratoses and was diagnosed with Cowden disease following the identification of a de novo PTEN variant. This case highlights acral keratoses as a clinical manifestation of Cowden disease and emphasizes the importance of differentiating them from palmoplantar keratoderma.
