Abstract
Abstract Topic Esophageal Cancer: Surgical Treatment of Esophageal Cancer – technique Video Description This video demonstrates an innovative reduced-port robotic approach to gastric conduit reconstruction during esophagectomy that bridges the gap between conventional multi-port robotic surgery and emerging single-port technologies. The technique uniquely combines robotic precision with direct tactile assessment through a single 5cm incision. What the Video Shows The comprehensive surgical demonstration includes: (1) Initial supra-umbilical incision with direct palpation and preservation of the critical right gastroepiploic vascular arcade; (2) Glove port assembly and strategic robotic arm positioning utilizing DaVinci Xi platform; (3) Complete robotic gastric mobilization including gastrocolic ligament division, lesser sac entry, fundal mobilization, and left gastric vessel ligation with lymphadenectomy; (4) Extracorporeal gastric tube construction with meticulous staple line reinforcement; and (5) ICG perfusion test.(4,5 not in the video) Why This Video is Unique and Important This technique addresses three critical challenges in minimally invasive esophagectomy: preventing iatrogenic gastroepiploic vessel injury through direct palpation (particularly crucial in obese patients and post-neoadjuvant cases), enabling superior conduit quality control through extracorporeal construction with potential for indocyanine green perfusion assessment, and reducing port requirements while maintaining full Xi platform capability including advanced energy devices unavailable on single-port systems. For the ISDE 2026 Congress audience, this video presents a practical, reproducible technique that surgeons can immediately implement without requiring new robotic platforms. It represents a evolution in minimally invasive esophagectomy, potentially reducing conduit-related complications while maintaining cosmetic and recovery advantages. The hybrid approach offers particular value for centers transitioning toward single-port surgery and for complex cases requiring maximal vascular preservation.
