Abstract
Abstract Topic Esophageal Cancer: Surgical Treatment of Esophageal Cancer – early outcomes and complications Background Prehabilitation, involving preoperative exercise, nutrition, and psychological support, aims to optimise patients prior to major surgery. Their impact on patients undergoing oesophagogastrectomy remains unclear - this study aims to assess the impact of prehabilitation on postoperative outcomes in patients undergoing oesophagogastrectomy. Methods A systematic review and meta-analysis was conducted in accordance with PRISMA guidelines. PubMed, Scopus, and Embase were searched for randomised controlled trials (RCTs) evaluating prehabilitation in adults undergoing oesophagogastrectomy. Data were analysed using RevMan v5.4. Results Eight RCTs involving 916 patients were included. Prehabilitation significantly improved preoperative 6-minute walk distance (6MWD) (mean difference [MD]: 34.52 meters, 95% confidence interval (CI): 17.77-51.26) and postoperative 6MWD (MD: 54.79 meters, 95%CI 36.66-72.92). Complications were significantly reduced (odds ratio [OR]: 0.48, 95%CI: 0.32-0.72) following prehabilitation. No significant differences were observed for severe complications (OR: 0.77, 95%CI: 0.43-1.39), length of stay (LOS) (MD: -0.85 days, 95%CI: -2.25-0.55,) or intensive care unit LOS (MD: -0.32 days, 95%CI: -0.71-0.07). Conclusion Prehabilitation enhances mobility and reduced postoperative complications following oesophagogastrectomy. This data supports its integration into the standardised preoperative care package for patients undergoing this procedure.
