Abstract
The integration of artificial intelligence (AI) into clinical documentation is reshaping infectious diseases (ID) outpatient practice. While natural language processing (NLP)-based scribing tools may improve efficiency and reduce administrative burden, they risk altering the illness narrative embedded within clinical notes. In ID settings, where diagnostic uncertainty is common, documentation can serve both as clinical reasoning and a therapeutic tool. This essay examines how AI-generated documentation may flatten narrative nuance and considers the implications of this shift in the era of open notes. It argues that the loss of interpretive, patient-centered language may diminish the therapeutic potential of the medical record. It argues that, although AI-assisted documentation likely offers efficiencies, preserving the clinician's role in interpreting and authoring the illness narrative remains an essential component of patient care.
