Abstract
Abstract Purpose To investigate the relationship between concussion symptoms and Buffalo Concussion Treadmill Test (BCTT) performance outcomes. Method We conducted a retrospective medical chart review of 98 diagnosed concussion cases that occurred in NCAA Division-I athletes (female=60, male=38, age=20.2±1.6) between August 2022-December 2025 at one institution. A sports medicine team member administered the BCTT to inform exercise prescription in accordance with the institution’s concussion management protocols. The 22-item Post-Concussion Symptom Scale was completed just prior to BCTT completion, with total number of symptoms (0-22) and overall symptom severity (0-132) serving as our independent variables of interest. BCTT guidelines for administration, minute-by-minute data capture, and termination were followed. Test duration (mins) and HR (bpm) and Borg RPE at termination were our dependent variable BCTT performance outcomes. Results Concussion cases had an average of 8.1±7.0 number of symptoms, 18.6±20.7 symptom severity, 165.2±20.9 bpm termination HR, 13.6±2.9 termination RPE, 17.8±6.4 mins test duration. A greater number of symptoms was associated with higher termination RPE (r=0.34, 95% CI 0.14-0.50, p<0.001) and shorter BCTT duration (r=-0.37, 95% CI -0.53-0.19, p < 0.001), but not termination HR (r=-0.16, 95% CI -0.35-0.04, p=0.12). Greater symptom severity was associated with higher termination RPE (r=0.39, 95% CI 0.21-0.55, p<0.001), shorter BCTT duration (r=-0.35, 95% CI -0.51-0.16, p<0.001), and lower termination HR (r=-0.21, 95% CI -0.40-0.01, p=0.04). Conclusions Higher symptom burden was associated with decreased exercise tolerance during the sub-acute phase of injury, emphasizing the value of symptom assessment alongside exercise prescription to guide individualized concussion treatment and return-to-sport progression.
