Abstract
Abstract Purpose Sport-related concussion (SRC) surveillance data are often drawn from emergency department or specialty clinic settings, which may underrepresent the full spectrum of injuries. Athletic trainer (AT)-based programs embedded within schools capture a broader range of SRC presentations. The current study characterized the demographics, injury characteristics, and symptom profiles of high school athletes with SRC evaluated by embedded ATs. Method A retrospective cohort study was conducted using a REDCap database of high school athletes diagnosed with SRC between the 2013 and 2024 across more than 25 schools and organizations. ATs documented initial evaluations and subsequent encounters through return-to-play. Descriptive statistics characterized demographics, medical history, injury characteristics, and Post-Concussion Symptom Scale (PCSS) scores. Results A total of 1,945 athletes (mean age 15.3±2.0 years; 67.9% male) were included. Most participated in contact sports (87.3%), with football (49.5%), soccer (21.2%), and basketball (14.0%) most common. Loss of consciousness and amnesia occurred in 5.4% and 4.8% of athletes, respectively. Mean initial PCSS was 15.2±13.9, with 55.4% of athletes reporting a PCSS of ≤14. Headache (68.3%) and dizziness (46.9%) were the most frequently endorsed symptoms. Conclusions The current AT-embedded, multi-school cohort provides a large characterization of SRC in high school athletes managed outside of traditional clinical settings. Findings highlight the symptom burden and recovery timelines in this population and may inform concussion management protocols and surveillance efforts at the interscholastic level.
