Abstract
Abstract Purpose Smart-device concussion testing tools, including Sway Medical (Sway), have a building empirical base but have not been compared with previously validated computerized concussion tests. The current study examines the relationship and clinical utility of Sway and ImPACT in high school athletes. Method 3 high schools completed Sway and ImPACT at baseline, post-injury (24-48 hours), and clearance testing (symptom-free). ATs were instructed to complete administration on the same day if possible. Differences based on time between administrations were examined if they occurred. Correlations (Pearson r) were completed at baseline between ImPACT and Sway. Post-injury comparisons were made qualitatively given the limited post-injury sample size. Results 403 athletes completed ImPACT and Sway baselines (46%female;M age=15.38, SD=1.27;n=244 ≤1week, 227≤1day, and 167 same-day). Sway Balance showed no correlation to ImPACT composites (p>.05); Reaction Time, Inspection Time, and Impulse Control had significant correlations to ImPACT composites (p<.01-p<.05) but small effects (R2≤.10); the Memory subtest showed significant correlations to ImPACT composites (p<. 01), with small-to-moderate effects (R2=.03-.15). No meaningful difference in correlation was observed based on days between administrations. A small sample (n=10) had baseline and acute post-injury data (<7 days) for both measures at similar administration times. Qualitative comparison showed overall post-injury decisions were the same whether based on Sway or ImPACT. Conclusions Sway correlations with ImPACT had minimal to mild effects, suggesting they are likely measuring different neurocognitive constructs. Sway qualitatively showed overall similar utility to ImPACT in the acute post-concussion setting. Further research should be focused on a larger sample size, particularly post-injury.
