Abstract
Abstract Purpose Adolescents with sport-related concussions(SRC) are at elevated risk of repeat injury. The Useful-Field-of-View Assessment(UFOV) measures visual processing efficiency, a modifiable cognitive domain that could be targeted to reduce sport-related injury risk. However, UFOV is not administered as part of current standard-of-care, and it is unclear how symptoms common post-SRC may relate to UFOV performance. This study characterized UFOV performance in adolescents post-SRC and evaluated associations with mood and post-concussive symptoms. Method Forty-four adolescents with SRC (Mdays_since_injury=28.52(30.42);Mage= 15.67(1.27),75% Female) completed UFOV (BrainHQ) and questionnaires assessing anxiety(GAD-7), depressive(PHQ-8), and post-concussive symptoms(Rivermead Post-Concussion Symptoms Questionnaire; RPQ) during a specialty concussion clinic visit. Greater GAD-7, PHQ-8, and RPQ scores reflect greater symptom burden. The UFOV involves three subtests that evaluate visual processing speed(UFOV1), divided attention(UFOV2), and selective attention(UFOV3), with lower scores indicating more efficient visual processing (milliseconds). Associations were assessed using Spearman’s rho. Results All participants achieved the best possible UFOV1 score. UFOV2 (M=48.53[22.95],Med=43.67) and UFOV3 (M=100.73[55.02],Med=86.67) showed greater variability. Anxiety and depressive symptoms were minimal (GAD-7: M=4.28[4.04]); PHQ-8: M=6.46[4.39]), and post-concussive symptoms were moderate (RPQ: M=20.98[10.83]). Greater anxiety symptoms were significantly associated with worse UFOV2 (ρ=0.32,p=0.04) but not UFOV3 performance (ρ=-0.15,p=0.34). We failed to detect significant associations between depressive or concussive symptoms with UFOV2 or UFOV3. Conclusions Findings demonstrate a ceiling effect for UFOV1 and suggest UFOV2 may be related to anxiety post-SRC. Future work with larger sample sizes should consider the role of demographic and injury-related characteristics in this association and consider how performance changes during clinical recovery.
