Abstract
Abstract Purpose A sport-related concussion is a traumatic brain injury (TBI) caused by a direct blow to the head, neck, or body, resulting in immediate or evolving signs and symptoms of TBI. By definition, no abnormalities are seen on standard structural neuroimaging studies, such as computed tomography or magnetic resonance imaging (MRI). If a trauma-related abnormality is identified after a sport-related head injury, the injury is classified as a mild TBI with neuroimaging evidence of structural intracranial injury. However, little is known about intracranial abnormalities visible on MRI after sport-related head trauma or concussions. Method This retrospective cross-sectional program evaluation study examined children, adolescents, and young adults who presented to the Mass General for Children Sports Concussion Clinic for clinical care. Individuals were included if they were new patients under age 30 and presented to the clinic during one fiscal year, between October 1, 2024, and September 31, 2025. Results Of 201 patients, 28 underwent head and brain MRI as part of their clinical care (13.9%), most of whom were referred by providers not affiliated with the concussion clinic. Of those, 12/28 (42.9%) had some abnormality (12/201; 6.0% of the total sample). Only one (0.5% of the total sample) had definitive trauma-related findings (i.e., subarachnoid hemorrhage and possible diffuse axonal injury). All remaining findings were incidental, including non-specific white matter hyperintensities, Chiari I malformation, and cysts (pineal/pituitary variants). Conclusions Brain MRI was infrequently obtained and rarely identified trauma-related intracranial abnormalities, supporting recommendations against routine imaging following sport-related concussion.
