Abstract
Abstract Purpose To examine the association between Buffalo Concussion Treadmill Test (BCTT) performance and recovery timelines in concussions sustained by collegiate athletes. Method We conducted a retrospective medical chart review of 98 diagnosed concussions sustained by NCAA Division I collegiate athletes [20.2±1.6 years old; 61.2%(n=60) female] between August 2022-December 2025. The BCTT was administered within 5 days of injury by an athletic trainer from the institution’s sports medicine team who documented test duration and termination heart rate outcomes. The sports medicine team used the BCTT termination heart rate to prescribe aerobic exercise for Step 2 of the Return-to-Sport strategy. For this study, recovery timelines were defined as days from injury to 1) self-reported symptom resolution and 2) return-to-sport (RTS). Results Mean termination heart rate was 165.2±20.9 beats per minute and mean test duration was 17.8±6.4 minutes. Median time to symptom resolution was 5 days (IQR:3-9) and RTS was 10 days (IQR:7-14). Cox proportional hazard models controlling for age, sex, and acute symptom scores demonstrated termination heart rate was not associated with time to symptom resolution (HR=1.00, 95% CI: 0.99-1.01, z=0.60, p=0.56) or RTS (HR=1.00, 95%CI:0.99-1.01, z=0.27,p=0.79). In contrast, longer test duration was associated with faster time to symptom resolution (HR=1.09, 95%CI:1.04-1.14, z=3.67, p=<0.001) and RTS (HR=1.08, 95%CI:1.03-1.13, z=3.25, p=0.001). Conclusions Longer BCTT duration, but not termination heart rate, was independently associated with faster concussion recovery timelines in collegiate athletes. These findings support the need for future prospective investigations of sub-acute BCTT performance metrics as potential prognostic indicators of concussion recovery.
