Abstract
Abstract Purpose To examine how diagnosed ADHD and stimulant medication use impacts baseline test performance in Division I collegiate athletes. Method A prospective study of collegiate athletes who completed preseason baseline testing was conducted. Participants self-reported diagnosed ADHD, current medications, and completed a baseline assessment battery (SCAT6/VOMS). Participants were grouped by ADHD and medication status (ADHD/medication, ADHD/no medication, no ADHD/no medication). Symptom severity, VOMS total score, VOMS change score, and SCAT6 total score were compared using a one-way multivariate analysis of variance (MANOVA).Follow-up analyses of significant findings using three univariate ANOVAs and Tukey’s tests were conducted. Statistical significance was set at p<0.05. Results 980 collegiate athletes completing pre-season baseline assessments (470 [48.0%] female; age=20.1±1.5 years) were included; 876 (89.4%) no ADHD/no medication, 53 (5.4%) ADHD/medication, and 51 (5.2%) ADHD/no medication. Results revealed a significant multivariate effect of group on baseline test performance, F(8,1920)=5.00, p<0.001, Wilks' Lambda=0.96, partial η2=0.02. Univariate ANOVAs revealed that ADHD and stimulant medication status was only significantly associated with symptom severity (p<0.001, partial η2=0.03). Follow-up Tukey’s tests found that the no ADHD/no medication group reported significantly less severe symptoms than ADHD/no medication (p<0.001, mean ∆ =-3.9) and ADHD/medication groups (p<0.001, mean ∆ =-3.4). No differences were found for medication status between the ADHD groups (p=0.91). Conclusions There was a significant effect of ADHD on baseline reported symptom severity. Individuals without ADHD reported significantly lower baseline symptom severity than those with ADHD, regardless of medication status. Clinicians should be mindful of ADHD when interpreting preseason symptom reporting during post-injury evaluation.
