Abstract
Abstract Purpose Examine the association between self-reported behavioral regulation strategies and primary concussion clinical profiles in adolescents with concussion. Method A retrospective chart review of adolescent patients (12-18yrs) seeking care for sport- or non-sport related concussions within 2-14 days of injury was conducted. All participants completed a clinical interview and multidomain assessment battery at their first clinical visit and were assigned a primary clinical profile (Migraine/Headache, Vestibular, Ocular, Cognitive/Fatigue, and Anxiety/Mood) based on clinicians’ interpretation of information gathered during the visit. During the clinical visit, participants completed a self-report, 4-point Likert scale (0-Never, 3-Most of the Time) questionnaire that assesses regulation of Sleep, Physical Activity (PA), Diet, Hydration, and Stress Management following their injury. Frequency for each domain was divided into HIGH (Most- or Some of the time) and LOW (Seldom or Never) regulation groups. A series of chi-square tests of independence were used to examine associations between HIGH and LOW regulation domains and primary concussion profiles. Results 1145 participants (14.98+1.67 yrs, 58% male) completed study measures 4.89 days (SD=2.88) following injury. Significant associations were observed for sleep regulation with Migraine/Headache profile (high=47.4%, low=36.4%), (χ2(1)=[5.13], p =.02) and Anxiety/Mood profile (high=2.6%, low=6.8%), (χ2(1)= [6.15], p= .01). PA regulation was associated with Vestibular profile (high=8.5%, low=12.6%) (χ2(1) =[3.96], p= .05). No other associations between behavioral regulation domains and primary clinical profiles were significant. Conclusions Self-reported behavioral regulation, specifically sleep and PA, are associated with distinct post-concussion presentations. Targeting sleep and PA regulation within the clinical management of concussion may help address specific post-injury presentations and improve recovery outcomes.
