Abstract
Abstract Purpose Limited research indicates that self-reported concussion history may be inconsistent. Therefore, we aimed to describe the consistency of self-reported concussion history among middle school (MS) student-athletes. Method Certified athletic trainers, part of the ACHIEVES Project, administered baseline concussion assessments (SCAT3, ChildSCAT5, and SCAT5) to MS student-athletes in a public-school district in Virginia between the 2015 and 2022 school years. De-identified concussion history data from 6,702 MS student-athletes were retrospectively analyzed using the first-last transition probability approach from 1,431 MS students (mean age = 11.73 years; standard deviation = 0.75; range = 10-14; 60.7% male) having 2+ concussion baseline assessments (median re-test interval = 12.5 months; standard deviation = 8.0; interquartile range = 11.8-23.7) between grades 6 through 8. Results Of the 1,431 student-athletes included in the sample, 153 self-reported a life-time history of at least one concussion. Among students reporting no concussion history at first assessment, 92% (95% CI: 0.9015-0.9317) maintained their response, while 8% (95% CI: 0.0683-0.0985) reported a history of concussion at last assessment. For students who reported a history of concussion at first assessment, 77% (95% CI: 0.6985-0.8307) maintained their response, while 23% (95% CI: 0.1693-0.3015) reported no history of concussion at last assessment. Conclusions Although most MS students consistently reported their concussion history, about 1 in 4 students who initially reported a prior concussion did not at follow-up, highlighting variability in longitudinal self-reporting. These findings emphasize the importance of utilizing a multi-informant approach when working with children and adolescents to ensure accurate acquisition of medical history for post-injury management.
