Abstract
Abstract Purpose The purpose of this study was to evaluate the effects of hormonal contraception (HC) on concussion recovery in female collegiate athletes. Method This prospective cohort study included 194 concussed Division 1 female athletes (average age 19.86[±1.28]) who self-reported using (n=99[HC]) or not using (n=95[NoHC]) a form of HC. All participants completed a baseline and symptom-resolution at rest (SRA) assessment following their concussion, which included the revised Head Injury Scale (HIS-r), Immediate Postconcussion and Cognitive Test (ImPACT), and Sensory Organization Test (SOT). Following a diagnosed concussion by an athletic trainer or physician, athletes completed the HIS-r daily until SRA. Once the athlete achieved SRA, they were administered the HIS-r, ImPACT and SOT. Mann-Whitney U Tests were used to compare days for date of injury (DOI) to SRA between groups. Paired-samples t-tests were used to evaluate within-group changes from baseline to SRA on all assessment measures (α=0.05). Results Within-group comparisons showed that both the HC and NoHC groups had improvements on the HIS-r, ImPACT composites, and SOT ratios when compared to baseline. While both groups improved, significant baseline-SF improvements were observed only in the HC group for ImPACT visual motor speed (p<0.001), reaction time (p=0.002) and SOT somatic ratios (p=0.007). There were no statistically significant differences in days for DOI-SRA (p=0.87) between athletes in the HC (median [IQR]: 10[9] days) and NoHC (11[10] days) groups. Conclusions While HC did affect performance on three variables, our results suggest that HC may not affect length of recovery after concussion or summary scores on HIS-r, ImPACT, and SOT.
