Abstract
Abstract Aims Emergency laparotomy (EmLap) is associated with high mortality and morbidity. However, its impact on post-EmLap quality of life (QoL), particularly among frail patients, remains poorly defined. This study aimed to evaluate the effect of frailty on QoL at 30 and 90 days post-EmLap. Methods Prospectively collected data from EmLap patients age ≥40 years between May'22 and July'23 at a district general hospital were analysed. Frailty was assessed using Clinical Frailty Scale, with scores >4 as frail. QoL was measured at 30 and 90 days post-EmLap using EQ-5D-5L, including utility and EQ-VAS scores. Changes across EQ-5D-5L domains were examined. Wilcoxon signed-rank test compared utility scores between timepoints, and Mann-Whitney U test assessed differences by frailty status. Statistical significant was set at p <0.05. Results 115 EmLap patients completed the EQ-5D-5L questionnaire. 10% were frail. From 30 to 90 days post-EmLap, there was a significant improvement in median EQ-VAS scores (70 to 80) and functional recovery, particularly in usual activities. Median utility scores improved significantly over time (0.850 to 0.916, p<0.001). Although frail patients had lower utility scores at both timepoints (0.717 and 0.922 at days 30 and 90) compared with non-frail patients (0.872 and 0.916), they demonstrated a significantly greater improvement in QoL between both timepoints (p=0.041). Conclusion QoL improves between 30 and 90 days post-EmLap. Despite poorer early post-operative QoL, frail patients experience clinically meaningful recovery by 90 days. These findings support a shift toward frailty-focused EmLap pathways, embedding geriatric expertise and targeted rehabilitation to optimise patient-centred outcomes.
