Abstract
Abstract Background Colorectal Malignancy with peritoneal metastasis is associated with a poor prognosis and high morbidity. Recent Meta-analysis published in colorectal disease 2025 has shown that Cytoreductive surgery may offer symptomatic relief and increase survival in selected patients. However these inventions are associated with significant peri-operative risk. This study reports peri-operative and short term outcomes for patient’s that underwent either Cytoreductive surgery or Tumour debulking surgery at a single centre. Methods A retrospective review was performed of patients undergoing tumour debulking or cytoreductive surgery for colorectal peritoneal disease between 2022–2025. Data collected included patient demographics, post-operative length of stay (LOS), complications graded using the Clavien–Dindo classification, 30-day mortality, and disease recurrence. Results Seventeen patients (10 Tumour debulking and 7 cytoreductive surgery) were included. Median age was 68 years. Median length of stay was 11 days (range 4–38 days). Post-operative complications occurred in 7 patients (41%) of patients. Clavien–Dindo grade I–II complications were observed in 4 patients, while 3 patients experienced major complications (grade III–IV). The 30-day mortality rate was 0%. Overall mortality as of January 2026 was 35%. Four patients had who underwent tumour debulking had died (40%), Two Cytoreductive surgery had passed away (28.5%). Median mortality time was of 11.3 months (range 4 – 22 months). Disease recurrence occurred in 4 patients at a median follow-up of 10 months. Conclusion Tumour debulking and cytoreductive surgery for colorectal peritoneal disease is associated with a significant complication burden and mortality risk even in carefully selected patients. These findings support the role of multidisciplinary patient selection and highlight the importance of counselling regarding peri-operative risks. Further research looking at quality of life for those undergoing primarily tumour debulking would be a valuable measurement and could help aid further decision making in these complex patients.
