Abstract
Abstract Aim To analyse post-operative and survival outcomes of patients who underwent surgery for cholangiocarcinoma over a 10-year period at a single tertiary HPB centre. Methods This retrospective single-centre cohort study included patients who underwent curative-intent resection for cholangiocarcinoma between 2014 and 2024. Patients were stratified by tumour subtype: intrahepatic cholangiocarcinoma (iCCA), perihilar cholangiocarcinoma (pCCA), and gallbladder cancer (GBC). Data collected included patient demographics, tumour characteristics and postoperative outcomes including overall survival. Statistical analyses were performed using SPSS, employing log-rank testing for survival analysis and Kruskal–Wallis χ² tests for group comparisons. Results A total of 106 patients underwent curative-intent resection during the study period with 33 for (iCCA), 33 (pCCA) and 40 (GBC). Median overall survival for the cohort was 44.4 months with a median follow up of 71.7 months. When stratified by tumour subtype, there was no significant difference in overall survival between the three groups (p = 0.651). Node-negative (N0) status was associated with improved overall survival, reaching statistical significance in the pCCA subgroup. Margin-negative resections (R0) were also associated with improved survival, with significance observed in the iCCA and GBC groups. Median length of hospital stay was 5 days for GBC, 7 days for iCCA, and 11 days for pCCA. Sixteen patients (15%) experienced Clavien–Dindo grade ≥III complications, including three 90-day mortalities, all occurring in the pCCA group. Conclusion This single-centre experience demonstrates perioperative and survival outcomes comparable to those reported in the literature and consistent with established prognostic factors for surgically managed cholangiocarcinoma.
