Abstract
Abstract Aims Emergency General Surgery (EGS) is the largest acute surgical specialty in the UK. Effective ambulatory care is essential in managing EGS patients, with blood results playing a crucial role in safe decision-making. Point-of-care blood testing (POCBT) may present an opportunity to streamline ambulatory pathways; however its role within EGS is unclear. This review aims to summarise existing literature on POCBT across all hospital settings, to assess its utility in EGS. Methods Systematic searches were performed in MEDLINE, Embase, Emcare and CINAHL from inception to January 2025, identifying all published studies reporting on POCBT in adult hospital settings. Data extraction included general study and participant characteristics, analyser details, tests performed, and study outcomes. Results were summarised descriptively. Results 36 full-text papers were included. Most were cross-sectional studies (n=28) and set in Emergency Departments (n=15). 30 POC analysers were reported across the studies, most commonly for FBC (n=18) or U&E (n=20) panels. Positive outcomes were often shown to be associated with POCBT, particularly around reliability (n=24), time savings (n=7), impact on clinical management (n=4), staff satisfaction (n=3) and overall cost savings (n=3). Some studies, however, did suggest the under-reporting of certain analytes with POCBT (n=10), concern at increasing staff workload (n=1), and hidden costs of implementation (n=1). Conclusion This review suggests that POCBT can deliver timely, reliable blood results, meaning it could be of benefit in EGS settings. A prospective pilot study may be the next appropriate step to assess this in clinical practice.
