Abstract
Abstract Aims Liver Function Test (LFT) monitoring is commonly performed post-operatively following colorectal surgery. Whilst the benefit of other blood tests is well evidenced, the utility of regular LFT testing in this cohort is unclear. A single LFT panel costs the laboratory at our institution £19.28. Our aim was to assess the frequency of LFT testing in colorectal patients following elective surgery, whether an indication was evident, and to establish whether abnormal results altered management. Methods A retrospective study reviewed patients who underwent elective surgery over a 4-week period in a tertiary colorectal unit. We calculated the number of LFT panels (excluding albumin) performed in the post-operative period, if they were abnormal, if an indication was clearly evident and whether results altered management. Results A total number of 269 tests were performed, costing £5,186. Of the 48 patients, 73% had post-operative LFTs and of those who had LFTs checked, only 54% had a clearly evident indication for monitoring. The median number of LFTs checked per patient was 5. Of the total number of LFT sets requested, 57% were normal. Of the 48 total patients reviewed, LFTs altered patient management. Conclusion The majority of cases tests were within normal limits. In cases in which they were deranged, it rarely influenced a change in management. More selective testing would reduce unnecessary cost and workload for the clinical team. We aim to re-audit these results after improvement of IT requesting systems and departmental education in attempt to optimise unnecessary laboratory testing and thus budget.
