Abstract
Abstract Aim Gallbladder polyps (GBP) are usually benign and commonly detected on imaging. However, they have a small risk of neoplastic transformation. Management is guided by polyp size, growth, and symptoms. This study reports our experience in surgical management of GBP and the correlation between radiological and histopathological findings. Methods A retrospective audit of all adult patient undergoing laparoscopic cholecystectomy was conducted at a single centre between January 2017 and October 2025. Patients were included if pre operative imaging had identified a GBP. Data collected included patient demographics, Charlson Comorbidity Index (CCI), indications for surgery, radiological and histological findings. Results Of approximately 1300 patients who underwent cholecystectomy, 51 patients were for GBP on imaging and were included in the final analysis. 58.8% were female, 45.1% of patients having CCI ≥3. Ultrasound demonstrated polyps ≥10 mm in 43.1%. Multiple polyps were presented in 54.9% of cases. MRI was used in 5 (9.8%) patients, with confirmed polyp detection in only one patient. 31 patients (60%) underwent surgery due to polyp size the remainder were experiencing symptoms attributed to gallbladder. Final histopathology revealed cholesterol polyps and cholesterolosis as the most frequent findings. Neoplastic lesions were identified in 5 patients (9.8%) 4 adenomas and 1 adenocarcinoma. These patients were all 65years or older, 4 were single polyps and 1 multiple and measured from 9 to 45mm. Conclusion Most gallbladder polyps are benign, with cholesterol-related pathology predominating. However, a small but clinically relevant risk of neoplasia exists. Ultrasound remains an effective first-line investigation.
