Abstract
Abstract Aims Endoscopic mucosal resection (EMR) of colorectal polyps is a developing technique for the management of possible colorectal malignancies. This study assesses the performance of Peterborough City Hospital’s (PCH) EMR polypectomy techniques against the national standard EMR rates and aims to identify factors that hinder the success of EMR over its proven benefits. Methods A retrospective analysis of all polypectomy cases undertaken in 2023 at PCH was conducted. Over 550 patients’ data, including endoscopy reports, analysed. Polypectomy techniques of EMR, cold and hot-snare compared for completeness and method of resection; en-bloc or piecemeal. Short- and long-term complications of perforation, bleeding and recurrence were compared along with any risk factors highlighted in challenging or complicated polypectomy procedures. Results Of the 550 patient data entries analysed, 50% underwent an EMR polypectomy, with an equal split between 2-week wait referrals and surveillance referrals. Most often, these were en-bloc resections. The vast majority of these had a complete resection success rate, with limited recurrences and even fewer significant short-term complications. Conclusion Highlighting the overall success of EMR, proves its values as a technique as well as an essential service required for the local population. It proves to be a competitive alternative in managing specific colorectal malignancies and therefore highlights the training requirements and demands for this technique. In the future, it will be important to evaluate the benefit of EMR across different sites, which will act as a learning guide for techniques, recognising challenging factors and identifying suitability of EMR for different cases.
