Abstract
Abstract Aims Acute kidney injury (AKI) is a common and serious complication of acute pancreatitis (AP), associated with increased morbidity, mortality and progression to chronic kidney disease. A systematic review was conducted to assess the predictive value of serum and urinary biomarkers in early detection of AKI in patients with AP. Methods A PRISMA-compliant systematic review was conducted across multiple online databases and clinical trial registries. Primary aim was to assess the predicative efficacy of the biomarkers (sensitivity, specificity, and area under the curve). The associated length of hospital stay, intensive care unit admission, mortality between AKI and non-AKI groups was assessed. Results Twelve studies included 3115 patients; six were retrospective and six were prospective. Altogether, 474(15.2%) patients developed AKI following admission with AP, with a higher incidence observed in males. The mean/median age of the non-AKI cohort in (n=10) studies was <60years whereas in the AKI cohort, four studies had a mean/median age of >60years. Out of fourteen biomarkers, urinary NGAL and serum cystatin-c were the most frequently analysed biomarkers across the studies. For serum cystatin-c, three studies had AUC >0.91, (p<0.05) and two studies reported AUC<0.80, (p>0.5) showing variability, however urine NGAL had a AUC>0.86, (p<0.005) across three studies deeming it a comparable biomarker. AKI was strongly associated with adverse clinical outcomes, including ICU admission and mortality across all studies. Conclusion Specific biomarkers have a promising role in the early prediction of AKI in patients with AP aiding in early treatment and therefore reducing negative outcomes.
