Abstract
Abstract Aims To compare sinus laser closure (SILAC) with Karydakis flap surgery in the management of pilonidal sinus disease, focusing on postoperative pain, hospital stay, recovery time, complications, and recurrence rates. Methods This prospective comparative study was conducted over an 18-month period. Sixty patients diagnosed with pilonidal sinus disease were enrolled and divided into two equal groups. Group A underwent sinus laser closure using a diode laser with a wavelength of 1470 nm and power of 10 watts delivered through radial fibers. Group B underwent Karydakis flap surgery. Patients were followed postoperatively and assessed for operative outcomes including postoperative pain, wound discharge, length of hospital stay, time to return to normal activities, complications, and recurrence. Results Both techniques demonstrated comparable recurrence rates during the follow-up period. Patients treated with SILAC experienced significantly less postoperative pain, shorter hospital stays, and earlier return to normal daily activities compared with those undergoing Karydakis flap surgery. However, postoperative wound discharge was more frequent in the laser group. No major intraoperative or postoperative complications were reported in either group. Conclusions Sinus laser closure is a safe and effective minimally invasive alternative to Karydakis flap surgery for pilonidal sinus disease. It provides similar recurrence rates with the advantages of reduced postoperative discomfort and faster recovery, although its higher cost remains a limitation.
