Abstract
Abstract Aims Appendicular tumours comprise a heterogeneous group of benign and malignant neoplasms, often presenting as acute appendicitis and diagnosed incidentally on histology following appendicectomy. This study analyses the histological subtypes, management strategies, outcomes, and challenges associated with appendicular tumours within our Trust. Methods A retrospective review was conducted of patients diagnosed with appendicular tumours at Mid and South Essex NHS Foundation Trust between 1 January 2015 and 31 December 2024. Patients under 15 years of age and those without histological confirmation were excluded. Results Among 2,623 appendicectomies, 149 patients (5.7%) were found to have appendicular neoplasms. Median age was 59.5 years (range 15–92), with a female predominance (M:F 1:1.57). Histological subtypes included low-grade appendicular mucinous neoplasm (LAMN) (34.2%), mucinous adenocarcinoma (32.2%), neuroendocrine tumour (NET) (27.5%), goblet cell adenocarcinoma (2.7%), and mucinous cystadenoma (3.4%). Definitive surgical management varied by histology, location, and stage of the tumour, ranging from appendicectomy alone to right hemicolectomy, sometimes combined with gynaecological resections. Adjuvant chemotherapy was administered in 40.4% of adenocarcinoma cases, including HIPEC in four patients. Synchronous caecal adenocarcinoma was identified on colonoscopy in three cases. Twenty-three patients with adenocarcinoma required tertiary referral. Five-year mortality for T4 adenocarcinoma was 50%, while no five-year mortality was observed for NET or LAMN. Conclusions Preoperative misdiagnosis, tumour rarity necessitating tertiary referral, and diverse histological subtypes requiring distinct surgical, oncological, and surveillance strategies, contribute to challenges in the management of appendicular tumours.
