Abstract
Medical training resources, including textbooks and board examinations, shape physician readiness for clinical practice. This commentary investigates whether educational prioritisation aligns with the real-world epidemiological burden of disease. By cross-referencing textual emphasis in a leading internal medicine textbook and examination blueprints from the UK (MRCP) and US (ABIM) with 2023 Global Burden of Disease mortality and prevalence data, we demonstrate a profound discordance. We observed no significant correlation between textbook volume and disease burden, while examination blueprints showed notable specialty-level mismatches. These findings suggest a 'curriculum lag', likely reflecting historical pedagogy rather than current clinical realities. We advocate for the integration of modern epidemiological data into future curriculum and assessment updates to ensure internal medicine training prioritises the primary drivers of population morbidity and mortality.
