Abstract
High- and very high-risk cutaneous squamous cell carcinomas are often histologically upstaged, following excision of the lesion, explaining most non-adherence to British Association of Dermatology (BAD) peripheral margin guidance. Incomplete excision was overwhelmingly due to deep margin involvement, while peripheral margins were rarely a problem. Findings suggest that current guidelines underemphasize deep surgical planning, and that improved preoperative assessment, use of Mohs surgery, and involvement of experienced multidisciplinary team (MDT) clinicians may enhance deep clearance and overall outcomes.
