Abstract
Spinal epidural lipomatosis (SEL) is a rare condition characterized by excessive fat deposition in the spinal epidural space. Although most cases are asymptomatic, severe SEL can cause lumbar spinal stenosis (LSS) and subsequent neurological deficits. Here, we report a case of LSS exacerbated by SEL. An 80-year-old man presented with a 1-year history of intermittent claudication and lower back pain. Magnetic resonance imaging revealed severe LSS at L3–L5 and excessive fat deposition in the spinal epidural space. Conservative treatment was ineffective, and the patient underwent L3–L5 laminectomy and partial resection of the epidural fat. Postoperatively, the patient's symptoms improved significantly. Histopathological examination of the resected tissue confirmed the presence of mature adipose tissue. This case highlights the importance of considering SEL as a potential exacerbating factor in patients with LSS. Surgical intervention can be effective in relieving symptoms and improving the quality of life in patients with LSS exacerbated by SEL.
