Abstract
Se estudiaron 18 pacientes ingresados con calazar. Se evidenció la mayor incidencia de la enfermedad en edades pediátricas y sexo masculino (61 %). Los síntomas más sobresalientes que estuvieron presentes en el 100 % de los pacientes, fueron: la fiebre prolongada y la palidez, acompañadas de esplenomegalia y anemia. Otros elementos clínicos-humorales presentes fueron: tos (44 %); hepatomegalia y distensión abdominal (39 %); íctero, edema y diarrea (17 %); ascitis, epistaxis, petequias, y sangramientos (11 %); leucopenia (94 %); con linfocitosis (100 %) y trombocitopenia (28 %). Se le realizó punción esplénica en busca de leishmanias a los 18 pacientes, y fue positiva en 15 de ellos (85 %); de los negativos, a 1 se le realizó punción medular que resultó positiva. Las complicaciones presentadas fueron las infecciones respiratorias (17 %) y hemorrágicas (11 %). La fiebre, la anemia y la esplenomegalia fueron los elementos clínicos predominantes. La evolución satisfactoria de todos nuestros casos confirmó la efectividad del glucantime como terapia de elección.
18 patients admitted with kala-azar were studied. There was a higher incidence at pediatric age and among males (61 %). The most marked symptoms that were observed in 100 % of the patients were: prolonged fever and pallor, accompanied with splenomegalia, and anemia. Other clinical-humoral elements were: cough (44 %), hepatomegalia and abdominal distension (39 %), icterus, oedema, and diarrhea (17 %), ascites, epistaxis, petechiae, and bleedings (11 %), leukopenia (94 %), with lymphocytes (100 %), and thrombocytopenia (28 %). Splenic puncture was performed to seek Leishmania in 18 patients. It was positive in 15 of them (85 %). One of the negative patients tested positive on undergoing medullary puncture. The complications observed were respiratory (17 %) and hemorrhagic infections (11 %). Fever, anemia, and splenomegalia were predominant clinical elements. The satisfactory evolution of all our cases confirmed the effectiveness of glucantime as an elective therapy.
