Abstract
Introducción: Las reintervenciones constituyen un grave problema que inquietan a los cirujanos y han inspirado múltiples estudios. Objetivo: Describir el comportamiento de las reintervenciones en el servicio de cirugía, se propuso realizar este trabajo. Método: Se realizó un estudio descriptivo, retrospectivo, de corte transversal de los pacientes reintervenidos en el servicio de cirugía general en el Hospital General Docente "Abel Santamaría Cuadrado" de Pinar del Río en los años 2006 y 2007. Se analizaron la edad, el sexo, indicación de la reintervención, tiempo entre la operación inicial y la reintervención, número de reintervenciones y la causa de muerte. El universo de estudio fueron los pacientes sometidos a cirugía mayor (3128) y la muestra quedó constituida por 108 pacientes que fue preciso reintervenir. Se realizó cálculo porcentual como método estadístico y se plasmaron los resultados en tablas. Resultados: El 69,38% correspondió al sexo masculino, y el 59,2% se hallaba en la séptima década de vida. El 67,34 % fueron operados de urgencia, predominando la oclusión intestinal mecánica y la litiasis vesicular. El 57,41% de las reintervenciones se realizó antes del séptimo día de la intervención inicial. Las causas principales de reintervención fueron: peritonitis residual (34,87%), colecciones intraabdominales (18,07%) y dehiscencias de sutura (14,71%). A medida que se incrementó el número de reintervenciones aumento la mortalidad y esta fue del 43,52% en los pacientes reintervenidos. Conclusiones: Mientras más precozmente se realice el diagnóstico de una complicación intrabdominal y más rápido se decida la reintervención menores serán las complicaciones y mayor la posibilidad de sobrevida del enfermo.
Introducction: Re-operations constitute a serious object of concerns to surgeons and have encouraged multiple studies. Objective: To describe the behavior of the re-operations in the surgical service. Method: A descriptive, retrospective, cross-sectional study choosing the re-operated patients in the service of surgery at "Abel Santamaria Cuadrado" University Hospital during 2006-2007. Pinar del Rio. Age, sex, and indication to perform the re-operation, period of time between the initial operation and the re-operation, number of re-operations and cause of death were the variables taken. The target group included the patients who underwent major surgeries (3128) and the sample comprised 108 patients requiring re-operations. Percentage terms were used as a statistical method and the results showed in tables. Results: 69, 38% belonged to male sex, 59, 2 % of the patients were in the seventies. Mechanical intestinal occlusion and lithiasis prevailed as emergencies 67, 34%. The 57, 41% of the re-operations were performed seven days after the initial surgery. The main causes of re-operations were: residual peritonitis (34, 87%), intra-abdominal collections (18,07%) and dehiscence of suture (14,71%). As the numbers of re-operations increased the mortality rate of the re-operated patients also increased (43,52%). Conclusion: as early the diagnosis of an intra-abdominal complication is performed and as rapid the decision of re-operations is made, complications will diminish and survival rates possibilities will augment.
