Abstract
Abstract Objective To evaluate the clinical efficacy of Peri-acetabular osteotomy (PAO) in the treatment of developmental dysplasia of the hip (DDH) complicated by osteonecrosis of the femoral head (ONFH). Methods A retrospective analysis was conducted on the clinical data of 7 patients (8 hips) with DDH complicated by ONFH who underwent PAO at our hospital between 2017 and 2023 to evaluate clinical and imaging outcomes. Results All patients successfully underwent surgery. Compared with preoperative values, at the final follow-up, the iHOT-12 score [(65.5 ± 13.0), (85.0 ± 25.0), P = 0.024] and Harris score [(63.8 ± 10.6), (81.9 ± 15.5), P = 0.013], hip extension ROM [(8.9 ± 2.3)°, (11.7 ± 3.5)°, P = 0.015], and hip internal rotation ROM [(11.1 ± 7.5)°, (22.2 ± 10.0)°, P=0.013] all increased significantly. Radiographically, compared with preoperative measurements, at the final follow-up, acetabular coverage [(66.1±5.7)%, (92.4±3.4)%, P<0.001], the Sharp angle [(45.4±2.2)°, (33.9±5.5)° , P<0.001], LCEA [(16.3±5.8)°, (43.4±3.2)°, P<0.001], and Tönnis angle [(18.0±6.5)°, (-7.0±5.6)°, P<0.001] all improved significantly. Conclusion In addition to improving the acetabular coverage in DDH, PAO can also shift the location of the necrotic area in ONFH to a non-weight-bearing zone, thereby promoting bone repair, improving hip function, and delaying the onset of arthritis.
