Abstract
Abstract Purpose To evaluate the short-term clinical outcomes of simultaneous arthroscopic hip rotator cuff repair and acetabular labral repair compared to isolated labral repair, while characterizing the clinical features of these concomitant pathologies. Methods We conducted a retrospective matched-cohort study of patients who underwent primary arthroscopic repair for combined acetabular labral tears and hip rotator cuff injuries between September 2019 and September 2023. These patients were matched 1:2 by sex, age (±2 years)), and BMI (±5 kg/m2) to a control group undergoing isolated labral repair. Patient-reported outcomes (PROs)—including the modified Harris Hip Score (mHHS), WOMAC, iHOT-12, and VAS pain scale—were assessed preoperatively and at a 2-year follow-up. Success was further defined by the proportions of patients achieving the Minimal Clinically Important Difference (MCID) and Patient Acceptable Symptom State (PASS). Results The study included 31 patients in the concomitant repair group (83.87% female; mean age 52.13 years) and 62 in the control group. Baseline demographics, imaging findings, and preoperative PROs were comparable between cohorts (P > 0.05). At 2 years postoperatively, both groups demonstrated significant improvements across all PRO measures compared to baseline (P < 0.001). There were no statistically significant differences between the groups regarding postoperative PRO scores or the rates of achieving MCID and PASS (P > 0.05). Conclusion Concurrent arthroscopic repair of hip rotator cuff injuries and acetabular labral tears yields favorable short-term outcomes comparable to isolated labral repair. These findings suggest that addressing both pathologies simultaneously is an effective surgical strategy for patients presenting with combined injuries.
