Abstract
Abstract Objective Aiming at the clinical problems of complex operation and insufficient osseointegration effect of the traditional multiple-screw fixation scheme in periacetabular osteotomy (PAO) for the treatment of developmental dysplasia of the hip, this study intended to design a PAO bone scaffold with both fixation, support and osteogenic functions, explore its biomechanical properties combined with different screw configurations, and provide theoretical basis and data support for the optimization of PAO surgical fixation schemes. Methods Firstly, a zoned PAO bone scaffold was designed, in which the dense structure undertakes the fixation function and the porous structure is responsible for support and promoting osseointegration. The scaffold was prepared by additive manufacturing and its mechanical property parameters were obtained. Subsequently, a finite element model of the pelvis after PAO was established to simulate the fixation scenarios of the bone scaffold combined with screws of different quantities and positions, and the differences in screw peak stress, displaced hip bone displacement and cortical bone stress of the resected bone block under different configurations were systematically compared. Result The results shows that when the bone scaffold was combined with 3 or more screws, the peak stress of the screws was significantly lower than that of the traditional 4-screw fixation scheme, and the displacement of the resected hip bone and the stress of the bone block were basically consistent with the traditional scheme; among them, under the configuration of the bone scaffold combined with Screw3 and 4 double screws, the peak stress of the screws was 9.4% lower than that of the traditional 4-screw scheme, and the fixation effect was equivalent to the traditional scheme; while when only the bone scaffold was used or combined with 1 screw, the displacement of the resected hip bone and the stress of the bone block increased significantly, which could not meet the requirements of fixation stability. Conclusion The conclusion of the study indicates that the PAO bone scaffold designed in this study can effectively disperse the screw stress, and can achieve even better biomechanical effects than the traditional 4 screws fixation when combined with 2-3 screws, which can simplify the surgical operation, reduce the use of internal implants, and ensure the fixation stability, providing an important reference for the clinical optimization of PAO surgical fixation schemes.
