Abstract
Abstract A 75-year-old male endurance athlete was evaluated at a sports cardiology clinic to assess the safety of continued intensive exercise in the presence of a known ascending thoracic aortic aneurysm (ATAA). He had adopted a healthier lifestyle at the age of 65 and subsequently completed more than 30 Ironman 70.3 races. In 2019, at the age of 75, transthoracic echocardiography demonstrated an ATAA measuring 52 mm, a bicuspid aortic valve, and moderate aortic regurgitation. The patient declined surgery and wished to continue endurance training. Following a shared decision-making process, continued participation in training under predefined blood pressure and heart rate limits, with regular imaging surveillance. During six years of follow-up, the aneurysm remained stable in size, and aortic wall thickness was consistently measured at 2.5 mm. According to Laplace’s law, the calculated circumferential aortic wall stress remained below estimated rupture thresholds, including during exercise. This case highlights that exercise counselling patients with ATAA should consider not only aneurysm diameter but also aortic wall thickness, and wall stress.
