Abstract
We report the first case of spontaneous regression of eruptive melanocytic naevi (EMN) following cessation of encorafenib, a BRAF inhibitor, in a patient treated for BRAFV600E-mutated metastatic colorectal cancer. The patient developed approximately 40 widespread naevi within two months of starting treatment, which showed near-complete involution within 12 months of drug withdrawal, confirmed on serial full-body photographic mapping and histopathology. This case supports the proposed mechanism of paradoxical MAPK pathway activation as the driver of BRAFi-associated EMN, and highlights the importance of dermatological surveillance in patients receiving BRAF-targeted therapies.
