Abstract
A 72-year-old man presented with a violaceous rash, confusion and profound hyperferritinaemia, later diagnosed as anti-MDA5 dermatomyositis. His course was rapidly progressive and complicated by encephalopathy and spontaneous retroperitoneal haemorrhage, reflecting a severe hyperinflammatory phenotype. This case underscores the importance of early recognition of this often fulminant form of dermatomyositis to improve outcomes.
