Abstract
Severe pembrolizumab-exacerbated psoriasis in a patient with disseminated stage IV melanoma posed a significant therapeutic challenge. Guselkumab resulted in rapid clinical improvement (PASI 12.3 to 3.1; DLQI 21 to 3), allowing pembrolizumab rechallenge without recurrence of severe psoriasis. This case highlights the potential role of IL-23 inhibition in immune checkpoint inhibitor-induced psoriasis while emphasising uncertainty surrounding oncological outcomes in this setting.
