Abstract
A 45-year-old man with advanced HIV infection (CD4+ T-cell count 24 cells/mm3) and chronic hepatitis B receiving tenofovir alafenamide presented with a one-month history of ulcerated plaques on the trunk and genitalia despite no recent sexual exposure. Histopathology showed ballooning degeneration of keratinocytes with eosinophilic intracytoplasmic inclusion bodies, and further investigations confirmed mpox infection; relapse after the initial course of tecovirimat required prolonged antiviral therapy together with initiation of antiretroviral treatment. This case highlights an atypical plaque-like ulcerative presentation of mpox in severe immunosuppression and suggests that concurrent tenofovir therapy may modify the incubation period or clinical expression of the disease.
