Abstract
A drenagem torácica é uma das terapias que deve ser adotada para o tratamento do paciente com trauma torácico. O objetivo principal da drenagem torácica é a remoção de gás ou líquido, usualmente sangue, do interior da cavidade pleural. Além disso, o dreno torácico induz reação tipo corpo estranho, promovendo a formação de fibrina intratorácica, que contribui para a vedação de pequenos orifícios na superfície pulmonar. Entretanto, a introdução de drenos torácicos em paciente acometido de fraturas de costelas é contra-indicada, pois no momento da introdução poderá ocorrer o deslocamento medial das esquírolas costais, incorrendo em lesão ao pulmão ou coração, podendo gerar hemopneumotórax. Em função disso, foi testada a possibilidade da introdução transdiafragmática, utilizando-se, para isso, seis cadáveres de cães. De um total de 36 introduções, apenas duas obtiveram êxito, o que reprova o uso desta técnica na rotina clínica em Medicina Veterinária.
The thoracic drainage is an important technique for the thoracic traumatized patient. Its main objective is to remove fluids or gazes from the pleural cavity through a thoracic tube. Besides, it induces fibrine production, because its reaction of foreign body. Fibrine has a crucial effect on closure of small lung ruptures, avoiding air leaking. Therefore, the thoracic drainage should be done very carefully in patients with rib fractures, due to its possibility of lung or heart lesion. The aim of this study was to test a new way of thoracic drain introduction, the transdiaphragmatic thoracic drainage, using six canine corpses. Within 36 tries, only two were successfully, which disapproves the use of this technique in Veterinary Medicine.
