Abstract
La incidencia real de lesiones quísticas malignas del tiroides no es bien conocida en Cuba. Estudiar este aspecto de las enfermedades de dicha glándula y evaluar la utilidad de la punción aspirativa con aguja fina en el diagnóstico de ellas constituyeron los objetivos del trabajo. Se operaron 104 pacientes por padecer lesiones nodulares del tiroides, como forma de presentación clínica, en un período de 4 años (1987 a 1990). La punción aspirativa con aguja fina se realizó a cada paciente en el quirófano inmediatamente antes de ser operado. El material obtenido mediante ésta y las piezas quirúrgicas se diagnosticaron por patólogos diferentes. Se obtuvieron un total de 45 (43,2 %) lesiones quísticas; de ellas 12(27 %) resultaron malignas. La punción aspirativa con aguja fina demostró una sensibilidad de 83 %, una especificidad de 97 %, con valores predictivos positivos y negativos de 90 y 94 % respectivamente. Las lesiones quísticas del tiroides pueden ser sitio de neoplasias malignas. Este tipo de lesiones no debe ser considerado siempre como sinónimo de benignidad
The real incidence of malignant cystic lesions of the thyroid is not well known in Cuba. To study this aspect of the thyroid diseases and to evaluate the usefulness of the fine needle aspiration biopsy in their diagnosis are the main objectives of this paper. 104 patients who suffered from nodular lesions of the thyroid as a form of clinical presentation were operated on from 1987 to 1990. All patients underwent fine needle aspiration biopsy in the operating room immediately after the operation. The material obtained as well as the surgical pieces were diagnosed by different pathologists. A total of 45 (43.2 %) cystic lesions was obtained. 12 (27 %) of them were malignant. The fine needle aspiration biopsy showed a sensitivity of 83 % and a specificity of 97 %, with positive and negative predictive values of 90 and 94 %, respectively. The cystic thyroid lesions may be the site of malignant neoplasias. This type of lesions should not be considered as a synonym of benignity
