Se desarrolló este trabajo para reflexionar sobre las enfermedades no transmisibles en su seguimiento a largo plazo y como situación de aprendizaje. Se discutieron las premisas, desafíos y derivaciones prácticas que impone ese modelo de seguimiento. Si un rápido diagnóstico y terapéutica acertados pueden ser suficientes ante una situación aguda de salud, muy diferente es con los enfermos crónicos, donde el diagnóstico implica cierta frustración y el tratamiento descansa en gran medida en su destreza y motivación, lo cual impone una situación de aprendizaje y, por tanto, nuevos desafíos a la gestión del cuidado. Para prevenir es importante el diagnóstico de salud de la comunidad y el carácter interdisciplinario e intersectorial de los programas de promoción y educación para la salud. Se impone en los proveedores de salud un “saber hacer” donde no basta la actualización clínica, son necesarias determinadas destrezas para ser eficaces en la situación de aprendizaje. Se requiere desarrollar estilos de vida y ambientes saludables; la unidad indisoluble de la gestión de cuidados y educación. Se plantea que el seguimiento a largo plazo impone un paradigma de cuidado diferente que fuerza a los proveedores de salud a sobrepasar la visión fisiológica del órgano dañado para centrarse en el hombre enfermo y ayudarlo a transitar por la vida sin minusvalía innecesaria y sin suplir el papel activo que éste debe desempeñar ante el desarrollo de su estado de salud.
This paper is aimed at reflecting on the non-communicable diseases and their long-term follow-up, as well as on the learning problem. The premises, challenges and practical consequences of this follow-up model are discussed. If a rapid diagnosis and a proper treatment may be enough in an acute health situation, it is very different with the chronic patients, where the diagnosis leads to certain frustration and the treatment relies mostly on their ability and motivation that requires some learning and, therefore, new challenges to care management. To prevent these diseases the health diagnosis of the community and the interdisciplinary and intersectorial character of the programs of promotion and health education are very important. Besides knowing how to do and being clinically updated, health providers need certain skills to be efficient in patient education. It is necessary to develop life styles and healthy environments and to attain the indissoluble unity of care management and education. It is stated that the long-term follow-up requires a different care paradigm that makes health providers to surpass the physiological vision of the damaged organ and to focus on the sick individual and help him to live without an unnecessary handicap and to play an active role in the development of his health status.
