Abstract
OBJETIVO: Conhecer o grau de eficiência com que funcionam os laboratórios clínicos de primeiro nível através de uma avaliação que integre: produtividade, qualidade e custos. MÉTODOS: De 52 laboratórios clínicos localizados na Cidade do México, selecionaram-se 10 laboratórios; utilizou-se o modelo de estrutura, processo e resultado de Donabedian, e a biometria hemática como rastreador. RESULTADOS: Os principais problemas encontrados foram: má distribuição de recursos humanos qualificados; material de vidro em más condições; técnica analítica inadequada; e recursos humanos com baixa produtividade. Esses problemas representaram aumento de 200% nos custos unitários em relação ao custo ideal. Apenas 50% dos laboratórios analisados tiveram resultados confiáveis. Somente 20% dos laboratórios trabalharam com eficiência. CONCLUSÕES: Para solucionar os problemas encontrados será preciso usar estratégias integrais que comprometam os diferentes setores, mas de solução difícil. Recomenda-se analisar o custo-benefício de criar um laboratório central e deixar os demais laboratórios com a função exclusiva de selecionar amostras.
OBJECTIVE: Assessment of productivity, quality and production costs and determination of the efficiency of top grade clinical laboratories in Mexico. METHODS: Ten laboratories were selected from among the total number (52) existing in Mexico City, and the Donabedian model of structure, process and results were applied. Blood count was selected as a tracer. RESULTS: The principal problems found were: inadequate distribution of trained human resources, poor glass material, inadequate analytic process and low productivity. These factors are reflected in the unit costs, which exceed reference laboratory costs by 200%. Only 50% of the laboratories analyzed generate reliable results. Only 20% of the laboratories studied operate efficiently. CONCLUSIONS: To solve the problems identified requires integral srategies at different levels. A specific recomendation for the improvement of quality and productivity is an assessment of the cost/benefit of creating a central laboratory and using the remaining sites exclusively for the collection of samples.
