Abstract
Se realizó un estudio retrospectivo de 54 pacientes con aneurismas múltiples del polígono de Willis, intervenidos quirúrgicamente en el Servicio de Neurocirugía del Hospital Clinicoquirúrgico "Hermanos Ameijeiras", desde enero de 1985 hasta diciembre de 1996. Se diagnosticaron mediante estudios angiográficos 121 sacos, de los cuales 115 se trataron quirúrgicamente en un tiempo y 4 en 2 tiempos. Dos sacos intracavernosos asintomáticos no fueron abordados. El abordaje de los 23 aneurismas situados bilateralmente fue unilateral en 19 casos (82,6 %) y bilateral en 4 (17,4 %). Se presillaron un total de 115 sacos y 4 fueron reforzados. Se determinó que el grupo de edad entre 41 y 50 años (33,3 %) y el sexo femenino (75,9 %) fueron los más afectados. La asociación de sacos situados bilateralmente en la circulación anterior fue la que más se observó (38,9 %). No se pudo demostrar que los resultados fueran dependientes del tipo de asociación, pero la combinación sacular anterior y posterior, así como los pacientes con multiplicidad sacular unilateral en la circulación anterior fueron los de mayor mortalidad (40 y 13,3 %), de lo que se infiere que el abordaje unilateral no influyó de forma negativa sobre los resultados, en los pacientes con aneurismas situados a ambos lados de la línea media. La mortalidad general fue de 9,25 % y la quirúrgica de 5,5 %
A retrospective study of 54 patients with multiple aneurysms in the circle of Willis, who had been operated on from January 1985 to December 1996 in the Neurosurgery Service of «Hermanos Ameijeiras» Hospital, was performed. Angiographic studies detected 121 sacs of which 115 were surgically treated in one attempt and 4 in two attempts. Two intracavenous asymptomatic sacs not approached. The approach of 23 bilaterally located aneurysms was unilateral for 19 cases (82.6 %) and bilateral in 4 (17.4 %). A total of 115 sacs were clamped and 4 reinforced. It was determined that 41-50 year age group (33.3 %) and females (75.9 %) were the most affected. The association of bilaterally located sacs in the anterior circulation was the most observed association (38.9 %). It could not be proved that the results dependen on the type of association but the highest mortality was recorded in anterior and posterior saccular combination and in patients with multiple unilateral sacs in the anterior circulation (40 % and 13.3 %). It can be inferred from this that the unilateral approach does not have a negative impact on the results achieved in patients with aneurysms at both sides of the midline. General mortality was 9.25 % and surgical mortality amounted to 5.5 %
